Adjutant not appearing?

While trying to mine items in the hidden sanctum(since Liar's Lair difficult hasnt dropped anything in the last 3 days), I've noticed that the Adjutant's spawn locations have changed. I've also noticed that about half the time, he no longer even appears. When approaching the one spawn location I know of, if one is perceptive, one can notice a red circle that appears almost onscreen, but then disappears when you try to run to see what it is.

If memory serves me well the Adjutant has regularly changed location. He can usually be found in the area close to either of the Gold Chests. He also doesn't always appear but that isn't new either.

Yeah, he can be found either near the fountains, somewhere else in the map (forget where), or sometimes even in the same room as that lvl 40 nefertari boss (which is a pain when you are soloing considering there are other nefs and monsters in the room)

He does appear randomly and is usually in one of the two spots mentioned above.

Are you sure you checked every corner of the Hidden Sanctum? There are several places where he appears: 1. near the big fountain; 2. in the southern part of the room where the Nefertari named boss is found; 3. there is a side path, on the left / western side of the map / dungeon, which is usually full with dragons and the Adjutant appears there sometimes (either near the northern part of the side path or in the lower end of it.
