There should be a NPC that for a fee destroys gems inside weapons and armor

no need, gems can be 'overwritten' just place the gem you want in an item as usual and pick which gem you'd like to replace.

u can destroy gems.once an item has all its gemslots filled.if you go to put another gem into a gemfilled item you will get a gem replace window and you can overrite one of the gems in there and replace it with another.

now currently though there is no way to reuse gems.to take them out and put them in another peaice of gear.once that oiece of gear is outleveled or you find a better pc of armor.your gems go tow aste and are stuck in that.
